This was brought up over the summer of 2013, so I drafted a rule to add to the books.
"3.1.1.1 [or maybe 3.7.3.2.11] Excessive players during active play: If the referee has ensured the correct number of active players are on the court at the start/resumption of play, and inactive/unwarranted players are discovered to be on the court for any reason, the the captain will receive a yellow card and the extra player(s) will be asked to leave the court. If the captain is not playing during the point in which the foul has been committed, the assistant captain will receive the yellow card. If the assistant captain is not playing during the point either, the other team will choose the recipient of the yellow card."
Another option would be that the captain receives the yellow card regardless of his "active player" status during the time of the infraction or not.
A third option is the captain [followed by assistant captain, then other team's choice] would be removed from play forcing the team who caused the infraction to lose an active player [just the same as a false start], along with the removal of the excessive player.
